How do we measure our success? Solutions Healthcare clinics providers of AADE accredited DSMES services, currently use three clearly defined markers to evaluate the success of our program. Firstly, we record process outcomes which measure patient retention and referral rate for six months. Secondly, we use reduction in A1c and weight and whether the patient receives an annual foot and eye exam. Thirdly, to evaluate the patient lifestyle behavior outcomes, we use S.M.A.R.T. goal (Specific Measurable Achievable Realistic Time bound) achievement tracking from the monthly visit notes of the DSME instructor. To assess patient satisfaction, we utilize an Instructor Evaluation form. This form is given to patients who complete our 12-month program. These are all proven assessments to evaluate the effectiveness of educational intervention. They contribute to long term positive health and clinical marker improvement.
